肺鳞30月，父亲永远地走了

5月22日：今天上午咳出好多血，看得我害怕，吃了3颗云南白药好点了。
3 @  ^6 O( O4 M验血报告出来了，在电脑上看的，胆红素、血小板 、凝血都是正常的。那咳血是什么原因呢？今天才知道老爸从5月2日起一直在吃云南白药，最近停了6天。
血常规忘了看了，但医生有说过是正常的。
今天做了增强CT，报告要明天出来，今夜注定要辗转反侧了。。。

  \- {/ v7 c" K' I( Z6 k# N, {1) CT示好转或稳定，则处理好咳血后继续用特，止疼药改成曲马多，用川贝枇杷膏处理肺热咳痰；
" I) \7 I* a% z2) CT示进展，则用吉西他滨或多西他赛化疗2周期，然后改用2992；

  Z8 J. Y& c; m上次最后一次化疗结束是去年11月16日，方案是紫杉醇+卡铂，3个月后进展；
9 z# s  h/ ?/ u/ [1 S/ H考虑已经用过紫杉醇，这次如果要化疗，准备先用吉西他滨，敢不敢加顺铂？ 还是就吉西他滨单药？目前体力尚可。
  ^4 c: a# j$ A! ^1 T3 o1 p; G

5月22日的CT报告如图，肺部肿瘤稍微增大、后肋骨转加重（胸骨骨质破坏稍好转）、产生胸水和心包积液；

+ e3 B! a7 v) S分析和教训：
1) 5月2日开始咳血，咳嗽增加，其实已经是有耐药的迹象了，但老爸人在老家，从未说过一直在吃云南白药；用镇咳药和云南白药盖住了症状，但盖不住肿瘤本身；
3 J2 o* N5 R; a0 [+ _3 G2）云南白药“可明显促进血管内皮生长因子(VEGF)的生成”，和抗肿瘤的方向背道而驰，临时止血可以，长期吃肯定不好；郁闷的是，在开始治疗前，还不能停。
/ w9 I3 W% l! \& v3) 特不能说完全耐药，毕竟肿瘤增大幅度很小，但不能再等了，要化疗打压；没准以后还有机会上特；

; K1 m3 L, N4 ]2 L9 M/ d  l$ U$ U; x周二开始天天和医院联系，要到周五下午才有床位，下周一开始治疗。我想还是下周一再去吧。

化疗还可以用泰索帝（多西他赛）、泽菲（吉西他滨）

184不敢用了，上次用了骨痛加重，副作用难以耐受（喝水都吐）、极度乏力；

另外，第五后肋骨肿块变大，靠近脊髓，风险很大。2月份已经做过姑息放疗，特有效期间肿块退缩，现在不知道该怎么办了。
2 X( p+ v5 Q: `* o唑来膦酸每月一针，胸骨骨质破坏稍有好转，后肋骨却没有。

也有资料显示凡德他尼不能延长总生存期。

! @7 f$ ]* s) u% a3) BATTLE报告中索拉菲尼对多种突变（EGFR、KRAS、VEGF、RXD/CycD1）和无以上突变的都有一定的有效率，尤其对KRAS突变有效率高达79%。
" d5 o( a! i( b( w; ^8 e5 S4) KRAS突变易腺癌为主，鳞的比较少，如果要试药，腺应该优先试用索拉菲尼（多吉美）。
5) 凡德总体有效率和厄洛替尼相当，但对KRAS突变、无其他突变的有效率为0，但厄洛替尼对KRAS突变、无突变都有一定的有效率。
